The Rural Health Transformation Program (RHTP) supports innovative approaches to strengthening rural healthcare through state-administered funding. For maternal health providers, it offers an opportunity to expand access to prenatal care through remote monitoring and other technology-enabled programs. Does your institution qualify for RHTP subgrants?

RHTP subgrants are distributed by states to licensed rural healthcare providers. If your organization serves rural patients in any of these categories, you are a potential subgrant recipient.

CAH

Critical Access Hospitals

Rural hospitals with 25 or fewer inpatient beds designated by CMS. CAHs are primary recipients of RHTP maternal care subgrants, particularly those in counties classified as maternity care deserts.

High RHTP priority category
FQHC

Federally Qualified Health Centers

Community health centers serving underserved rural and urban populations. FQHCs providing OB or prenatal services are eligible RHTP subgrant targets, particularly those serving Medicaid-predominant patient panels.
